THE MISSION of Faith Regional
Health Services is to provide all people
with exemplary medical services. We
do that by providing high-quality,
affordable health care that is accessible
throughout the 13 counties we serve.
The hospital’s strategic plan for the
delivery of these services is based on
the needs of patients in our service area
throughout northeast Nebraska and how
best to coordinate that care. Population
health management takes that process
and commitment to the next level.
Population health management is
an industry term that means we, as
health care providers, are responsible
for improving the overall health care
results of defined groups (or popula-
tions) of patients by gathering infor-
mation about health care consumers,
better identifying risk factors, educat-
ing patient populations, better tracking
procedures and outcomes, and reduc-
ing costs. We use evidence-based best
practices that touch all phases of care.
By involving everyone from hospitals
to providers to health plans and espe-
cially you, the patient, throughout the
continuum of care, we can positively
influence the health outcomes of our
population one person at a time.

with multiple health care agencies and
health departments in our region as
we worked to
identify the top
health needs of
our population.

Integration of health care delivery is a
key way that we are addressing popula-
tion health management. As a regional
provider, Faith Regional continues to
deliver a wide spectrum of primary care
providers and specialists that can attend
to the vast health needs of our popula-
tion, and we have the technology and
resources to positively impact your
health care now and into the future.
