Trying to remember all the details
about your procedures, tests, medica-
tions and other important health in-
formation can be difficult. But a new
online feature called Faith Regional
Health Connect can help.
Faith Regional Health Connect
is a secure patient portal that gives
you 24/7 access to information
about your visits to Faith Regional.
(Information about visits to Faith
Regional Physician Services clinics
will be available in the future.) It’s a
convenient way to manage your health
information on your own time.
By creating a Health Connect ac-
count, you will be able to access your
information sooner, instead of waiting
on a call from your doctor’s office.
If you’ve been a hospital patient or
visited a Faith Regional physician,
your visit summary will be available
on Health Connect within four days.
Health Connect also gives you the
capability to use secure
email to contact Faith
Regional. You can
use Health Connect’s
secure messaging
function to ask ques-
tions regarding your
information.
Health Connect is launching in two
phases: Phase I (beginning in July)
and Phase II (beginning in 2015). In
Phase I, information from inpatient
visits and discharge summary docu-
ments will be available. Additional test
results and information will be added
in Phase II, such as laboratory tests;
radiology reports (not including im-
ages); outpatient visit summary docu-
ments; and medical directives, living
will and medical power of attorney
documents.
When you are admitted as a patient
at Faith Regional, your information is
entered into the Health Connect sys-
tem and is accessible to your provider.
Once you complete the registration
process via email and online, you
will be able to log in and access your
account. If you decide you would no
longer like to be enrolled in Faith
Regional Health Connect, an opt-out
form can be submitted and your ac-
count can be removed.
Health Connect puts your care in-
formation together in one secure place
that makes it easy for you to share
with whomever you choose, including
doctors, family members and caregiv-
ers. Health Connect encourages you to
take a more active and involved role in
managing your health.
How secure is
Health Connect?
It’s very secure! Special features
keep your information secure
and prevent your records from
being accessed by anyone not
authorized to view them. Health
Connect technology has added
security features, such as audit
logs, which track who has ac-
cessed your records and when.
Data encryption provides an
even higher level of security.
However, in the unlikely event
that improper access occurred,
the law requires that you be noti-
fied, and Faith Regional would
immediately take steps to correct
the problem.
