and health systems are mandated by

the Affordable Care Act, along with state and/or local regulations, to conduct a

community health needs assessment (CHNA) every three years. This assessment

must include a broad group of agencies working together to identify what the

health concerns are, financial and other barriers, types of prevention programs

available, and social factors. Once the CHNA is completed, a plan to address the

health concerns is developed.

Over the next three years, Faith Regional Health Services’ Community Health

Improvement Plan (CHIP) will focus on two key areas that significantly affect the

health of our communities:

access to care

and

obesity

. Our goal with access to

care is to make health care accessible to all people within our service area.

Obesity is on the rise, and studies indicate it is the causing agent for other dis-

eases, such as heart and pulmonary issues and diabetes, and it contributes to the

rising cost of health care. For obesity, three goals were identified:

• 

Increase knowledge about risk factors impacted by obesity.

• 

Increase physical activity in adults and youth.

• 

Provide insight into healthy food alternatives and quick recipes.

One way we plan to reach youth is by teaching them how to perform CPR!

In addition to the CHIP, Faith Regional will continue to look at ways we can

improve population health through our occupational health program with busi-

nesses, by continuing to invest in advanced technology that connects us regionally

with academic medical centers, and through our nationally recognized quality

and safety initiatives. Providing you with coordinated care through all phases of

your life and having the medical staff and services available locally will allow us

to improve access to care and reduce obesity and disease. Faith Regional is com-

mitted to providing the level of care you need today and well into the future.
